As a patient of UCSF Health, you have the right to:
- Be treated with courtesy, dignity and respect
- Have your property, beliefs, preferences, and values respected
- Have reports of pain believed and be given all information about pain and pain relief measures
- Receive information in a way you can understand
- Exercise these rights regardless of your age, race, color, origin, religion, sex, disability, economic status, educational background, gender identity, marital status, or other category protected by law
- Know the name and title of the UCSF Health personnel who will be providing your care and identify personnel through proper identification
- Be involved in decisions regarding your care and overall care plan
- Be informed of the purpose, frequency and outcomes of the services provided, including any modifications to the care plan and any limitations
- Refuse care or treatment and be told of the consequences of that choice
- Be informed of your rights under state law and provide advance directives for the purpose of designating a decision maker if you are unable
- Choose a healthcare provider and expect us to coordinate care with your provider. As relevant, receive notice of transfer to another provider
- Expect privacy and confidentiality of your patient records and a copy of your records at your request
- Be informed, verbally and in writing, in advance of care being provided, of the charges, including payment for care expected from third parties and any charges for which you will be responsible
- Receive information regarding any financial relationships between us and other providers
- Voice grievances about treatment or care without fear of reprisal and with timely investigation
- Examine and receive an explanation of the bill
- Be advised on the organization's policies and procedures regarding the disclosure of clinical records
- Be fully informed if you are asked to participate in any clinical research
- Understand the purpose and scope of the patient management program and request information, including changes in, or termination of, the patient management program
- Provide personal health information to the patient management program only in accordance with state and federal law
- Identify the Pharmacy staff, including their job title, and speak with a staff member's supervisor or other health professionals if requested
- Decline participation, revoke consent, or dis-enroll in the patient management program at any point in time
Patient Responsibilities
- Give correct and complete information about your health, past illnesses, hospital stays, medications, allergies, and other important medical information
- Be a part of your care plan and make sure it is up to date
- Follow your updated care plan
- Be an active participant in the care you get from our providers and pharmacy
- Ask for more information when you do not fully understand the care being provided to you and notify the pharmacy of any concerns about the care or services provided
